Waiver of Liability
The undersigned hereby requests, of the City of Bothell Parks & Recreation, use of the facilities described on the park reservation form and certifies that the information in its request is correct. The undersigned agrees to exercise the utmost care in the use of these facilities and property and by signing below applicant hereby agrees to indemnify, defend and hold harmless the City, its elected and appointed officials and employees while acting within the scope of their duties, from any and all claims, demands and causes of action of any kind or character, foreseen or unforeseen, for damages including but not limited to personal injury, death or property damage as well as the cost of defense costs, court costs, witness and attorneys' fees, in any way arising out of the granting of this permit or arising out of the applicant's use of the public area or other premises permitted by this permit, except for damages arising out of the City's sole negligence. Applicant expressly waives his/her immunity under title 51 of the Revised Code of Washington, the Industrial Insurance Act, for injuries to his/her employees and agrees that the obligation to indemnify, defend and hold harmless provided for in this paragraph extends to any claim brought by or on behalf of any employee of the applicant.
Refund Policies
Before you reserve a facility, you should read and understand the following refund policies: Full refund/credit if request is made more than thirty (30) days prior to the reservation. | 50% refund/credit if request is made fifteen (15) to thirty (30) days prior to the reservation. | No refunds/credits issued if request is made less than fifteen (15) days prior to reservation. | 100% refund if reservation is cancelled by Bothell Parks & Recreation.
Recreation Programming
Hold Harmless Clause
I, the undersigned, parent/guardian, assume all risks and hazards incidental to participating in the activity and do hereby waive, release, absolve, indemnify and agree to hold harmless the City of Bothell, their supervisors, participants and instructors for any claim arising out of any injury to myself/child. I, the undersigned, parent/guardian of the participant, am fully aware of the potential dangers and risks inherent in this activity, including physical injury, death or other consequences that may arise or result directly or indirectly from participation in this activity.
Photo/Video Release
I grant full permission to use any photographs, videotapes, motion pictures, recordings or any other record of this program for promotion of the Bothell Parks & Recreation Department.
Resident Discount Fee
Bothell Parks and Recreation strives to provide quality and affordable recreation opportunities to the community. In recognition that Bothell citizens support our Parks and Recreation services through their city taxes, the City offers its residents a discount on some fee-based recreation programs. City of Bothell Parks and Recreation will make every effort to provide reasonable accommodation for people with special needs. Please contact us three (3) weeks prior to the class starting to discuss accommodations.

City of Bothell Parks and Recreation strives to provide the best possible recreational programs and to ensure customer satisfaction. If you are dissatisfied with the quality of the program, facility or instructor, you should contact the staff prior to the second class or within two days following a one day class so that your concerns can be quickly addressed.

Nondiscrimination Policy
The City of Bothell complies with the State of Washington's "Fair Play in Community Sports Act" (Chapter 467, 2009 Laws, effective date July 26, 2009) that prohibits discrimination against any person in a community athletics program on the basis of sex.
